FAQ – Frequently Asked Questions
What do I need to bring to my first session?
You need to bring the Intake Form filled out which you can download from this website on the Home page along with Release of Information Insurance, Notice of Privacy Practices, and Disclosure Statement filled out and signed where appropriate which can be downloaded from my home page. You should be prepared to bring the names of your doctors, their telephone and fax numbers, and names and amounts of any medications you are taking. You must also bring proof of income if you are seeking a sliding fee. Please bring your insurance card if you are using insurance to pay for your sessions. Please be prepared to pay the co-pay or coinsurance portion of the fee. I accept checks, cash, and credit cards.
How much do services cost?
Weekly sessions lasting 60 minutes are $120 per session and 45 minute sessions are $100 per session. The intake and assessment session is $160. If you would like a free consultation it is done over the phone and lasts about 10 minutes. After the free consultation phone calls, other than to change appointments, are billed by the quarter of the hour at the following rates: 15 minutes at $30, 30 minutes at $60, 45 minutes at $100 and 60 minute sessions are $120. Please note that your insurance company may not pay for phone consultations.
When is payment due?
Payment for services and copayments are due at the end of every session. I file insurance claims for clients on a monthly basis. It is the client’s responsibility to call their insurance company before they make an appointment with me to see if my services are covered and if they have met their deductible and what their co-pays are.
What type of payments do you take?
I take most insurance plans, cash, and checks, as well as Visa, Master Card, and Discovery credit cards
Do you accept insurance?
Yes I accept insurance, however, before you make an appointment with me it is your responsibility to call your insurance company and find out the following: if they pay for my individual services, if you have mental health benefits, if you have a deductible and has your deductible been met, how many sessions does your insurance allow per calendar year, will you owe co-pays or co-insurance, and does your insurance pay for out of network benefits if I am not in their network. By filling out my Intake Form you will have all of this information. I will collect copays and the full fee if your deductible has not been met at the end of each session.
What if I have no insurance and can’t pay the full fee?
I save limited space for sliding fee clients. However, you must furnish proof of your income either with a current 1040 form, your last pay stub, or your unemployment stub to qualify for a sliding fee.
